STEPS TO COMPLETING YOUR ATHLETIC REGISTRATION

Registration paperwork can be picked up and turned into the athletic office or you can download the forms from this site. (See Forms link on left menu bar)

Prior to the date of practice, students MUST PROVIDE PROOF OF A SPORTS QUALIFYING PHYSICAL EXAM, DONE WITHIN THREE (3) CALENDAR YEARS (June 2018) of the end of the season. Check with the athletics office (763-504-8533) or the school nurse (763-504-8509) to confirm that a current sports qualifying physical is on file before the sports practice season starts. If the student does not have a copy of a sports physical on file with the school nurse, the parent/guardian should call their doctor/clinic and get a copy. If the doctor’s office needs a form to fill out, a copy of the MSHSL Sports Qualifying Physical exam form may be picked up in the athletic office or download on the CHS website. If the student has not had a physical in the last three (3) years, then the student must get a physical. It is a Minnesota State High School League rule that all students participating in sports must have a current up-to-date Sports Qualifying Physical on file with the school at the time of registration. (The physical must state whether the student is able to participate in sports.)

IMPORTANT: Students will not be allowed to register for a sport if they have any outstanding fines or obligations. This may include: athletic, cafeteria, classroom and Media Center fines, etc. All fines/obligations must be cleared. Please check with the Bookkeeper to see if you're on the fine list. NO EXCEPTIONS!

IMPORTANT: Registration forms and fees must be completed and turned into the athletic office one week before the official practice start date. Until all of the above tasks are completed, students WILL NOT be allowed to try-out or practice. NO EXCEPTIONS!

IMPORTANT: Registration Fees – Fees are due at the time of registration. Registration fees are $210 per sport per season with a family max of $700.00. If a family applies and qualifies for a reduced or free lunch program with the school district’s Nutrition Office (763-504-8049) the athletic registration fees will be as follows:

Free lunch program ($50.00)
Reduced lunch program ($100.00)

NEW THIS YEAR! If you qualify for Free or Reduced lunch, you MUST bring in the letter you received from the district confirming your free/reduced status in order to receive benefits.
